New Study Suggests Bariatric Surgery May Reduce the Risk of Breast Cancer

or many years, bariatric surgery has been recognized as one of the most effective treatments for severe obesity and obesity-related conditions such as type 2 diabetes, hypertension, obstructive sleep apnea, fatty liver disease, and metabolic syndrome. Today, the scientific evidence continues to grow, showing that the benefits of metabolic and bariatric surgery extend well beyond weight loss.

A recently published systematic review and meta-analysis in Scientific Reports adds another important piece to that growing body of evidence. The study suggests that patients who undergo metabolic and bariatric surgery (MBS) may experience a lower incidence of breast cancer compared with individuals living with obesity who do not have surgery.

What Did the New Study Find?

The study, titled “An Updated Systematic Review, Meta-Analysis, and Network Meta-Analysis of Breast Cancer Incidence after Metabolic and Bariatric Surgery,” analyzed data from:

  • 17 comparative studies
  • 343,531 patients who underwent bariatric surgery
  • 1,587,413 control patients who did not undergo surgery

This makes it one of the largest analyses to date examining the relationship between bariatric surgery and breast cancer risk.

The researchers found that patients who underwent metabolic and bariatric surgery had approximately a 25% lower risk of developing breast cancer compared with similar patients who did not have surgery (Hazard Ratio 0.75, 95% Confidence Interval 0.70–0.81).

Among studies that included only women, the findings were remarkably similar, demonstrating a 26% reduction in breast cancer incidence.

Why Could Bariatric Surgery Influence Cancer Risk?

Obesity is now recognized as a major risk factor for several types of cancer, including breast cancer.

Excess body fat is not simply stored energy—it functions as an active endocrine organ that produces hormones and inflammatory molecules capable of influencing cancer development.

Researchers believe several mechanisms may explain why metabolic surgery appears to reduce cancer risk:

  • Reduction in chronic inflammation
  • Improved insulin sensitivity and lower insulin levels
  • Favorable changes in estrogen metabolism
  • Reduced production of inflammatory cytokines
  • Improvements in metabolic health beyond weight loss alone

These biological changes occur after metabolic surgery and may help create an environment that is less favorable for cancer development.

Sleeve Gastrectomy and Gastric Bypass Both Showed Benefits

One particularly interesting finding was that both of the most commonly performed bariatric procedures—

  • Sleeve Gastrectomy
  • Roux-en-Y Gastric Bypass

were associated with lower breast cancer incidence compared with patients who did not undergo surgery.

Importantly, the analysis found no evidence that one procedure was superior to the other regarding breast cancer prevention. This reinforces what we already know: both operations are excellent metabolic procedures when selected appropriately for the individual patient.

What About Premenopausal and Postmenopausal Women?

The researchers also examined whether menopausal status influenced the findings.

The reduction in breast cancer incidence appeared strongest among premenopausal women, while the benefit in postmenopausal women did not reach statistical significance.

This does not necessarily mean bariatric surgery has no benefit after menopause. Rather, it highlights the need for additional research to better understand how hormonal changes influence cancer risk after surgery.

An Important Point About the Evidence

One aspect of the study that deserves emphasis is the quality of the research.

Although the results are encouraging, the authors clearly state that the available evidence remains observational. In other words, researchers observed lower cancer rates among patients who had bariatric surgery, but they cannot yet prove that surgery directly caused the reduction.

Future prospective studies will help clarify:

  • Which patients benefit the most
  • Whether timing of surgery matters
  • How long the protective effect lasts
  • The biological mechanisms responsible

This is an important distinction because good medical decisions should always be based on balanced interpretation of the evidence rather than exaggerated claims.

Another Reason to View Bariatric Surgery as Metabolic Medicine

For many years, obesity was viewed primarily as an issue of body weight.

Modern medicine now understands obesity as a complex chronic disease affecting metabolism, hormones, inflammation, cardiovascular health, liver function, and even cancer risk.

This is why the term Metabolic and Bariatric Surgery (MBS) has become the preferred description worldwide. These procedures do much more than reduce the size of the stomach—they improve the body’s metabolic function in ways that can positively influence long-term health.

The potential reduction in breast cancer incidence adds to a growing list of documented health benefits that already includes improvements in:

  • Type 2 diabetes
  • High blood pressure
  • Obstructive sleep apnea
  • Dyslipidemia
  • Non-alcoholic fatty liver disease
  • Overall life expectancy

What This Means for Patients

Breast cancer prevention should not be considered a primary reason to undergo bariatric surgery. Surgery remains indicated for the treatment of obesity and obesity-related metabolic disease in appropriately selected patients.

However, studies like this remind us that treating obesity may also influence many aspects of long-term health that extend far beyond the number on the scale.

Every new high-quality study helps us better understand the broad impact of metabolic surgery and reinforces what many healthcare professionals have observed for years: improving metabolic health can have profound effects throughout the body.

When discussing bariatric surgery with patients, my goal is always to provide evidence-based recommendations tailored to each individual’s medical history, health goals, and long-term well-being. Research such as this supports the importance of viewing metabolic surgery as a comprehensive treatment for chronic obesity—not simply as a weight-loss procedure.
